About The Position

Highpoint Health Winchester with Ascension Saint Thomas is part of Lifepoint Health, a diversified healthcare delivery network. As a Rehabilitation LPN joining our team, you're embracing a vital mission dedicated to making communities healthier. We are a 12-bed inpatient rehabilitation unit and the only acute inpatient rehab option in the area, serving a diverse population with a wide range of diagnoses. Our team practices a strong interdisciplinary approach, allowing nurses to work closely with therapy, physicians, and case management to deliver comprehensive, patient-centered care. We maintain a supportive work environment with an average nurse-to-patient ratio of 4–5:1, promoting quality care and professional growth.

Requirements

• Current Tennessee or compact state LPN license
• Graduate from a Practical Nursing program
• AHA Basic Life Support (BLS), within 30 days of hire
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.
• LPN License for the State of Tennessee.
• Graduate of a Practical Nursing program.

Nice To Haves

• One-year previous experience preferred.

Responsibilities

• As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
• Under the direction of an RN or Rehab Charge Nurse, contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
• Assists RN with development of plan of care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
• Works as an advocate for the physical and emotional well-being of the patient.
