Licensed Practical Nurse (LPN) / Registered Nurse (RN) – Pediatric Home Health
About The Position
Angels of Care Pediatric Home Health is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide one-on-one, in-home care for medically fragile children and young adults in Uniontown, Pennsylvania, and surrounding areas. The role focuses on building meaningful relationships with patients and families while delivering high-quality, individualized care in the home setting. Angels of Care is a nurse-founded, clinically led organization that has served the medically fragile community since 2000. The position offers full-time, part-time, and PRN opportunities, with flexible scheduling based on availability. Shifts of 8, 10, 12, or 16 hours are available, including day, night, and weekend shifts. Nurses receive clinical and operational support, paid training, simulation lab experiences, and continuing education. Pay is listed as $35+ per hour, and the role is referral bonus eligible with an uncapped referral bonus. Requirements
- Active, unrestricted Pennsylvania Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- License must have been acquired at least one year before application.
- Ability to provide care in a home setting.
- Strong communication and clinical decision-making skills.

Nice To Haves
- Pediatric or home health experience.
- Experience with high-acuity skills such as tracheostomy, ventilator, or G-tube care.
Responsibilities
- Provide skilled nursing care in a home health setting.
- Perform ongoing patient assessments and clinical evaluations.
- Administer prescribed medications, treatments, and therapies.
- Deliver care in accordance with physician orders and the plan of care.
- Coordinate care with families, providers, and care teams.
- Educate patients and family members to support improved outcomes.
- Implement preventive measures to ensure patient safety and quality care.
- Respond appropriately to changes in a patient’s condition.
