About The Position

Abundance Day Center & Services is seeking compassionate, dependable, and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide high-quality nursing services to individuals with developmental disabilities in their homes and community settings. The LPN/RN provides skilled nursing care to individuals receiving services through Abundance Day Center & Services. Nurses will work directly with individuals in their homes to provide medical care, monitor health conditions, educate caregivers, and ensure services are delivered in accordance with physician orders, nursing standards, Medicaid requirements, and Ohio Department of Developmental Disabilities (DODD) regulations.

Requirements

  • Hold a current Ohio LPN or RN license in good standing.
  • Graduate from an accredited nursing program.
  • Maintain current CPR and First Aid certification.
  • Pass a criminal background check.
  • Possess a valid Ohio driver's license and reliable transportation.
  • Carry current automobile insurance.
  • Demonstrate excellent communication and organizational skills.
  • Be able to work independently with minimal supervision.
  • Maintain professionalism and confidentiality.

Nice To Haves

  • Experience working with individuals with developmental disabilities.
  • Home health or community nursing experience.
  • Experience with catheterization, wound care, medication administration, and chronic disease management.
  • Knowledge of Medicaid Waiver Services and DODD regulations.

Responsibilities

  • Administer prescribed medications and monitor for effectiveness and side effects.
  • Perform medication set-ups and medication management.
  • Provide catheter care, catheter insertion, and catheter changes as ordered.
  • Perform wound assessments and wound care according to physician orders.
  • Monitor vital signs and overall health status.
  • Perform nursing assessments and document findings.
  • Monitor chronic medical conditions.
  • Provide diabetic care, including blood glucose monitoring and insulin administration (as ordered).
  • Administer injections as prescribed.
  • Assist with tube feeding care (if applicable).
  • Perform tracheostomy care (if applicable).
  • Educate individuals, families, and caregivers regarding medications, treatments, and health management.
  • Collaborate with physicians, therapists, case managers, and interdisciplinary team members.
  • Recognize changes in health status and notify physicians or supervisors promptly.
  • Respond appropriately to medical emergencies.
  • Maintain accurate nursing documentation and electronic records.
  • Maintain infection control standards and universal precautions.
  • Ensure all nursing care complies with state and federal regulations.
  • Travel between client homes as scheduled.
  • Perform other nursing duties as assigned.
  • Provide safe, ethical, and compassionate nursing care.
  • Follow physician orders accurately.
  • Ensure medications are administered correctly and documented.
  • Maintain infection prevention and control practices.
  • Complete nursing documentation promptly and accurately.
  • Monitor for medication reactions or changes in condition.
  • Maintain communication with supervisors regarding changes in client status.
  • Respect each individual's dignity, privacy, and independence.
  • Maintain professional boundaries.
  • Participate in required trainings and continuing education.
  • Follow all DODD rules, Medicaid waiver requirements, and nursing standards of practice.
  • Protect individual rights and maintain person-centered care.
  • Maintain HIPAA confidentiality.
  • Report Major Unusual Incidents (MUIs) and Unusual Incidents (UIs) according to DODD policy.
  • Complete required documentation accurately and on time.
  • Follow each individual's Individual Service Plan (ISP) and Nursing Delegation Plan, when applicable.
  • Maintain current licensure and required certifications.
  • Complete all required annual trainings and competency reviews.
  • Follow all infection control and medication administration policies.
  • Promote health, independence, dignity, and community inclusion.
