About The Position

SmithLife Homecare is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. In this flexible role, you will provide skilled nursing services to clients in their homes, supporting safe, high-quality care and promoting independence. Working under the supervision of a Registered Nurse (RN), you will implement individualized care plans, monitor client conditions, and support families throughout the care journey.

Requirements

Active LPN license in Maryland and/or Washington, DC (in good standing)
Graduate of an accredited practical nursing program
Strong clinical judgment, communication, and documentation skills
Ability to work independently and adapt in a home-based setting

Nice To Haves

Minimum 3 years of clinical nursing experience preferred
Experience in home care, geriatrics, or chronic disease management preferred

Responsibilities

Deliver skilled nursing care in accordance with the established Plan of Care
Administer medications, injections, and treatments as prescribed
Monitor and document client condition, including vital signs and response to care
Observe and report changes in physical, emotional, or cognitive status
Assist with clinical procedures including wound care and infection prevention
Support clients with activities of daily living as needed
Educate clients and families on care needs and disease management
Maintain accurate, timely documentation in accordance with agency standards
Collaborate with RNs, care coordinators, and interdisciplinary team members
Follow all infection control protocols and agency policies
