About The Position

Eventide on Eighth Care Center is currently seeking a Licensed Practical Nurse (LPN)- PRN (As Needed). This position offers 12-hour Day Shift (6:30 am- 7:00 pm) or NOC shift (6:30 pm- 7:00 am) availability. The role requires availability to work a minimum of 24 hours each month and includes working two weekend shifts (Saturday/Sunday) every month. Eventide will reimburse the fee to acquire a border state license upon hire for ND nurses. A $5 an hour NOC shift differential and a $2.00 an hour Weekend shift differential are added to the base wage. Responsibilities

  • Assess residents’ needs and health status through physical assessments, observations, and interviews.
  • Provide individual attention, encouraging each resident’s ability to maintain or attain the highest physical, mental, and psychosocial well-being.
  • Observe and follow residents’ plans of care, updating and making appropriate changes whenever necessary.
  • Administer medications and treatments and monitor both for effectiveness.
  • Provide direct nursing care to residents whenever necessary and according to established nursing policies, procedures, and standards.
  • Comply with rules and regulations as set forth by the Minnesota or North Dakota Department of Health and the Center for Medicare/Medicaid services.
  • Provide accurate documentation in the residents’ electronic medical records according to established nursing policies, procedures, and standards.
  • Assist in orientation/training of new team members.
  • Report resident needs, problems, and observations regarding residents’ condition(s) to the Charge Nurse and/or attending physician.
  • Ensure adequate staffing levels for a scheduled shift and correct inadequate staffing appropriately.
  • Monitor the activities and functions of subordinate staff, which may include participating in the disciplinary process.
  • Ensure that work responsibilities of subordinate staff are completed thoroughly and accurately within scheduled shifts and according to established resident care policies, procedures, and standards.
  • Provide information, education, and support for assigned residents and their families.
  • Communicate the needs of residents to resident care staff in the unit, as appropriate.
  • Interact with physicians and other health care facilities as needed, according to the needs of the residents.
  • Continually update nursing skills and knowledge by attending in-services, continuing education programs, and staff meetings.
  • Coordinate and participate in resident care conferences and communicate information effectively to families.
  • Assume responsibility for promptly reporting any incidents or evidence of abuse, neglect, or violation of residents’ rights per policy.
  • Manage electronic timecards daily.
  • Carry out or assist with other tasks in addition to the duties listed in this job description as occasionally required.
