About The Position

Family Centers is seeking a caring, dedicated and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. Family Centers Health Care, a FQHC (Federally Qualified Health Center), provides primary healthcare, dental, and mental health services to children and adults living in Greenwich and Stamford with an emphasis on the special population living in or near public housing. The LPN provides clinical, administrative, and operational support to healthcare providers in an ambulatory setting. This role involves delivering high-quality, patient-centered care; supporting care coordination; managing medication and vaccine inventory; and ensuring efficient clinic operations in compliance with all regulatory and organizational standards.

Requirements

  • Completion of an accredited Licensed Practical Nurse program
  • Current LPN license in the state of Connecticut
  • Current CPR/BLS certification
  • Minimum of 2–4 years of related clinical experience in an ambulatory or healthcare setting
  • Experience with medication and vaccine inventory management
  • Knowledge of infection prevention and control practices
  • Strong communication, organizational, interpersonal, and customer service skills
  • Ability to work on-site 5:00pm - 8:00pm on rotating Tuesdays and Thursdays

Responsibilities

  • Provide clinical support during examinations and minor procedures, ensuring the delivery of exceptional care to patients and their families.
  • Perform patient intake, including vital signs, medical history reconciliation, and screening assessments, while maintaining precise records in the electronic health record (EHR) system.
  • Administer medications and immunizations as directed by providers and in accordance with standing orders and scope of practice.
  • Maintain accurate inventory of medications and vaccines, including ordering, receiving, and cold chain management in compliance with CDC and manufacturer guidelines.
  • Perform and document point-of-care waived tests (e.g., glucose monitoring, urinalysis, rapid tests) and maintain all associated laboratory logs.
  • Provide vital patient education regarding preventive care, chronic disease management (diabetes, hypertension, asthma), and medication adherence.
  • Participate in team-based care coordination, including daily huddles, patient outreach, and facilitating follow-up appointments and referrals.
  • Support specialty clinics and programs by addressing barriers to care and promoting continuity of care through proactive patient engagement.
  • Handle clinical communications, including phone calls and electronic messages, escalating patient concerns to the RN or provider as appropriate.
  • Ensure the proper functioning of medical equipment, manage clinical supplies, and ensure exam rooms are clean and prepared according to infection control protocols.
  • Participate in quality assurance and population health initiatives (e.g., UDS, HEDIS) to monitor service and operational efficiency.
  • Sustain a neat workspace and ensure strict adherence to HIPAA, OSHA, CLIA, HRSA, and Joint Commission guidelines.
  • Respect patient/client autonomy and dignity, demonstrating expertise in clinical testing and professional communication with the healthcare team.
  • Fully adhere to organizational policies, procedures, standing orders, and clinical protocols.
  • Perform other duties as assigned.
