As an LPN in our Primary Care Clinic, you will work alongside physicians, nurse practitioners, and clinical staff to deliver compassionate, patient-centered care. You will play an important role in supporting preventive care, managing chronic conditions, assisting with treatments and procedures, and promoting overall wellness. In this position, you will help create a positive patient experience while ensuring high-quality care in a collaborative outpatient setting. Under the supervision and delegation of a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.

Requirements

  • Graduate of a Practical Nursing program.
  • Current LPN to practice in the state.
  • BLS required

  • Previous Primary Care Clinic experience preferred

Responsibilities

  • Contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care; may administer medication.
  • Documents patient care provided.
  • Assists RN with development of plan of care.
  • Assist patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient status and responses to interventions with RN, physician, patient, and family.
  • Advocates for the physical and emotional well-being of the patient.
  • Participates in performance improvement and continuing education.
  • Provides quality patient care and works flexible shifts/units as needed.
  • Maintains regular and reliable attendance.
  • Performs other duties as assigned.
