About The Position

The Winchester Post Acute unit provides care for patients transitioning from hospital to home that can provide services such as post-surgical recovery, chronic illness management, and rehabilitation, helping them regain independence to return to the community. The multidisciplinary team of nurses, therapists, physicians, and support staff collaboratively work together to deliver a personalized care plan that also supports healing and emotional well-being. The unit also emphasizes patient education, family involvement, and continuity of care that makes patients feel seen, heard, and empowers them in their recovery. Requirements

  • Current Tennessee or compact state LPN license
  • Graduate from a Practical Nursing program
  • AHA Basic Life Support (BLS) within 30 days of hire
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment and take appropriate action.
  • LPN License for the State of Tennessee
  • Graduate of a Practical Nursing program

  • One-year previous experience preferred

Responsibilities

  • As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.
  • Under the direction of an RN or Post Acute Charge Nurse, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Works as an advocate for the physical and emotional well-being of the patient.
