Requirements

  • Active Florida LPN or RN license in good standing
  • Reliable transportation, valid driver’s license, and auto insurance
  • Current hands-on CPR certification (or willingness to complete)
  • TB test (PPD or lab work within 6 months) or chest X-ray within 5 years (or willingness to obtain)
  • Level 2 AHCA background screening (or willingness to complete)
  • Strong communication skills, professionalism, and reliability

Nice To Haves

  • Home care experience preferred but not required
  • Strong clinical judgment and attention to detail
  • Ability to work independently in a home setting

Responsibilities

  • Provide skilled nursing care in the client’s home in accordance with the plan of care
  • Monitor changes in patient condition
  • Administer medications and treatments as ordered
  • Support clients with chronic conditions and post-hospital care
  • Educate clients and family members on care needs and safety
  • Collaborate with physicians and the care team
  • Maintain accurate and timely documentation in WellSky app
  • Communicate proactively with the office regarding patient status or concerns
