Requirements

Active Florida LPN or RN license in good standing
Reliable transportation, valid driver’s license, and auto insurance
Current hands-on CPR certification (or willingness to complete)
TB test (PPD or lab work within 6 months) or chest X-ray within 5 years (or willingness to obtain)
Level 2 AHCA background screening (or willingness to complete)
Strong communication skills, professionalism, and reliability

Nice To Haves

Home care experience preferred but not required
Strong clinical judgment and attention to detail
Ability to work independently in a home setting

Responsibilities

Provide skilled nursing care in the client’s home in accordance with the plan of care
Monitor changes in patient condition
Administer medications and treatments as ordered
Support clients with chronic conditions and post-hospital care
Educate clients and family members on care needs and safety
Collaborate with physicians and the care team
Maintain accurate and timely documentation in WellSky app
Communicate proactively with the office regarding patient status or concerns
