This role is a great fit for nurses who enjoy one-on-one patient care, prefer a home environment over a facility setting, and are confident working independently while still having support from a dedicated office and nursing team. We offer flexible scheduling with opportunities that may include day visits, evening visits, overnight shifts, weekend availability, short skilled nursing visits, and ongoing case assignments when available. Requirements

Active Florida LPN or RN license in good standing
Reliable transportation, valid driver’s license, and auto insurance
Current hands-on CPR certification, or willingness to complete
TB test, PPD, or lab work within the last 6 months, or chest X-ray within the last 5 years
Level 2 AHCA background screening, or willingness to complete
Strong communication skills, professionalism, and reliability

Nice To Haves

Home care or private duty nursing experience preferred, but not required
Strong clinical judgment and attention to detail
Ability to work independently in a private home setting

Responsibilities

Provide skilled nursing care in the client’s home according to the plan of care
Monitor client condition and report changes or concerns promptly
Administer medications and treatments as ordered
Support clients with chronic conditions, post-hospital needs, and ongoing care needs
Provide client and family education related to care, safety, and condition changes
Communicate with physicians, families, and the care team as appropriate
Maintain accurate and timely documentation in the WellSky app
Work independently while following agency policies, care plans, and nursing direction
Communicate proactively with the office regarding patient status, scheduling, or care concerns
