Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Maryland.
  • Minimum of one (1) year of nursing experience in long-term care, skilled nursing, rehabilitation, or a related healthcare setting.
  • Strong knowledge of nursing documentation, resident assessment practices, and care planning.
  • Excellent organizational, analytical, and communication skills.
  • Ability to manage multiple priorities while meeting deadlines.
  • Ability to read and speak English.

Nice To Haves

  • Previous MDS Coordinator or MDS experience.
  • Experience with PointClickCare or other electronic health records systems.
  • Knowledge of quality measures and regulatory compliance requirements.

Responsibilities

  • Coordinate and complete the Minimum Data Set (MDS) assessment process in accordance with federal, state, and organizational regulations.
  • Collaborate with interdisciplinary team members to gather assessment data and ensure comprehensive resident evaluations.
  • Assist in the development and updating of individualized care plans based on resident needs and assessment findings.
  • Monitor assessment schedules and ensure timely completion and submission of all required documentation.
  • Review clinical documentation for accuracy, completeness, and regulatory compliance.
  • Support quality measures and performance improvement initiatives.
  • Serve as a clinical resource for MDS-related regulations, documentation requirements, and reimbursement processes.
  • Work closely with nursing leadership to promote excellence in resident care and regulatory compliance.
