About The Position
We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide high-quality hospice care to patients in home and facility-based settings . This role focuses on comfort, dignity, and emotional support for patients and their families during end-of-life care.
Requirements
- Active New Jersey LPN license (required)
- Strong communication and clinical skills
- Compassionate, patient-centered approach
- Reliable transportation and ability to travel within assigned counties
Nice To Haves
- Hospice or end-of-life care experience preferred (not required)
Responsibilities
- Provide direct nursing care to hospice patients in home or facility environments
- Monitor patient condition and report changes to the care team
- Administer medications and treatments as ordered
- Offer compassionate emotional support to patients and families
- Maintain accurate and timely documentation
- Collaborate with RNs, physicians, and hospice care teams
