Practices in accordance with the standards, scope and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) for a Licensed Practical Nurse (LPN) as well as within a client and family centred care model.The LPN works collaboratively across multiple sites, clinics, and services, or on outreach/home based visits within an integrated health service delivery model and interdisciplinary care team, to provide holistic care with primary care clients. The LPN cares for clients and families who are experiencing health challenges across the primary care continuum. In situations where client care needs are not stable or predictable, the LPN seeks direction/guidance from a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Nurse Practitioner (NP), or Physician (MD). The LPN works closely with the RN/RPN/NP/MD in direct care activities using a team-based care approach, such as assessing, planning, implementing, evaluating and documenting client care as part of the team and collaboratively with other Community Care Services. The LPN establishes, maintains and enhances therapeutic relationships based on respect.
