About The Position
The Care Team Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team! This is an excellent opportunity for a nurse who is passionate about providing hands-on patient care while supporting individuals and families through every stage of the hospice journey. As a Hospice LPN, you'll work alongside an experienced interdisciplinary team to deliver skilled nursing care, monitor patient needs, and help ensure patients receive the comfort, dignity, and support they deserve. Requirements
- Active Licensed Practical Nurse (LPN) license
- Previous nursing experience required
- Strong clinical assessment, communication, and patient care skills.
- Ability to work independently while effectively collaborating with an interdisciplinary team.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
- Current CPR certification.
- Current TB testing in accordance with agency policy.
Nice To Haves
- hospice, home health, long-term care, or skilled nursing experience preferred.
- Experience with electronic medical records (EMR) preferred.
Responsibilities
- Provide skilled nursing care under the direction of the Registered Nurse while following each patient's individualized plan of care.
- Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team.
- Assist with symptom management, medication administration, treatments, and patient comfort measures.
- Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR).
- Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care.
