Requirements

  • Graduate of an accredited school of Nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
  • Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
  • Valid driver's license, reliable transportation and an acceptable driving record.

Responsibilities

  • Provide skilled, age-appropriate nursing care to hospice patients in homes and residential care facilities in accordance with New Hampshire Board of Nursing standards and Hospice best practices.
  • Educate patients, families, and caregivers about the Hospice philosophy, available services, care expectations, and bereavement support.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, patients, caregivers, and the interdisciplinary hospice team.
  • Perform comprehensive patient assessments and ongoing reassessments, documenting care accurately and thoroughly.
  • Deliver treatments and nursing interventions while monitoring patient progress and adjusting care plans as needed.
  • Empower patients and caregivers through education that promotes comfort, safety, and confidence in care.
  • Collaborate closely with physicians, nurses, social workers, chaplains, volunteers, and other healthcare professionals to ensure coordinated, high-quality care.
  • Participate in interdisciplinary team meetings, patient care conferences, quality improvement initiatives, and agency committees.
  • Attend agency in-service trainings and continuing education opportunities to support professional growth.
