About The Position

This position involves 75% desk work and 25% face-to-face assessments, offering less time on your feet and more time coordinating quality care. The schedule is primarily Monday-Friday with only one rotating on-call weekend each month. Make a meaningful impact supporting individuals with developmental disabilities in your community.

Requirements

Licensed Practical Nurse (LPN) certified in New Mexico
At least 1 year of experience working with individuals with disabilities
Must pass TB test and background check
Must have own reliable transportation, valid NM driver’s license, insurable driving record and proof of current liability insurance
Must be able to sit, stand, walk, bend, stoop, carry, and perform keyboarding/typing tasks frequently
Must be able to squat, reach above shoulder(s), kneel, balance, push, pull and lift 50 lbs. occasionally

Nice To Haves

Experience with severe/profound disabilities
Experience with feeding tubes (jejunostomy/gastrostomy) preferred

Responsibilities

Create personalized healthcare plans that truly support each individual’s needs
Train and support staff so they feel confident providing care
Conduct nurse visits and attend medical appointments with clients
Participate in ISP & IDT meetings, ensuring each person’s voice is heard
Maintain accurate medication and treatment records to keep care safe and consistent
