About The Position
SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled care to patients throughout Mesa County. This position offers flexible scheduling and the opportunity to build a caseload over time. You'll work independently in patient homes while receiving support from an experienced team of clinicians dedicated to delivering high-quality, patient-centered care designed to improve quality of life.
Requirements
- Current Colorado Licensed Practical Nurse (LPN) License
- Valid driver's license, reliable vehicle, and auto insurance
- Strong communication and accurate documentation
- Ability to work independently and adapt professionally in home health environments
Nice To Haves
- IV Certification preferred; not required
Responsibilities
- Provide skilled nursing services in patient homes
- Perform wound care, medication support, and patient monitoring
- Observe and report changes or concerns promptly
- Document visits accurately and timely in the EMR
- Collaborate with RNs, therapists, caregivers, families, and providers
- Support patient education and individualized care plan goals
