About The Position

A qualified Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. Care is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. This position is required to take on-call shifts on nights, weekends and holidays as assigned. This position is bonus-eligible.

Requirements

  • Completion of an LPN program recognized by the State Board of Nursing.
  • One (1) year experience as an LPN.
  • Holds a current LPN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an LPN in the state the team member is working.
  • Valid driver’s license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance.
  • Ability to exercise initiative and independent judgment
  • Acceptance of philosophy and goals of this agency

Nice To Haves

  • Experience in Home Health preferred.
  • Successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency program from another organization.
  • Program equivalency recognized by the SC State Board of Nursing.
  • Candidates enrolled in an accredited LPN to RN transition program.

Responsibilities

  • Implements the nursing care plan for each patient.
  • Provides nursing services, treatments, and diagnostic and preventive procedures according to the plan of care as assigned.
  • Initiates preventive and rehabilitative nursing procedures as appropriate.
  • Prepares equipment and materials for treatments.
  • Assists the RN in performing specialized procedures.
  • Observes aseptic technique as required.
  • Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
  • Evaluates with the registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse.
  • Maintains accurate and complete records of observations, treatments and care of patient.
  • Prepares clinical and progress notes.
  • Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  • Assists the patient in learning appropriate self-care techniques.
  • Participates in patient care conferences and discusses the need for involvement of other members of the health team, such as physical therapist, speech therapist, occupational therapist, and medical social worker.
  • Submits any changes in schedule to clinical manager on a daily basis.
  • Understands and adheres to established policies and procedures.
  • Attends staff meetings, patient care conferences, and in-services as scheduled.
  • Participates in medical record audits as assigned.
  • Performs other duties as assigned.
