About The Position
Lake Prince At Home, Home Health and Hospice is seeking LPNs for their growing team. Lake Price Woods At Home is a non-profit organization that prides itself on serving the community at EveryAge. They cover the South Hampton Roads area, including the Lake Prince retirement community.
Requirements
- Unencumbered Virginia license as a Licensed Practical Nurse.
- Graduate of an accredited school of nursing.
- One year of nursing experience with Home Health preferred.
Responsibilities
- Under the direction of the RN Case Manager, provides nursing services/ patient care activities and accommodation services in accordance with standards, regulatory guidelines, and licensure requirements.
- Follows the plan of care while providing services.
- Communicates with Manager changes and collaborates with patient/family member/physician or Nurse Practitioner as needed or directed.
