Requirements

Current Missouri or Compact LPN license.
Excellent assessment and communication skills.
Strong organizational and time management abilities.
Compassionate, patient-centered approach to care.
Valid driver's license and reliable transportation.

Nice To Haves

Home health experience is a plus, but we welcome nurses who are eager to learn.

Responsibilities

Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans.
Perform comprehensive patient assessments and monitor changes in condition.
Educate patients and families to promote confidence and independence at home.
Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team.
Maintain accurate documentation using our AI-supported charting technology.
Participate in quality improvement initiatives and ongoing professional development.
