Licensed Practical Nurse, LPN - Home Health
About The Position
As a Licensed Practical/Vocational Nurse in Home Health, you play a critical role in helping patients heal, recover, and maintain independence in the place they feel most comfortable - their home. You bring consistency, compassion, and hands-on care to every visit, supporting both patients and the broader clinical team. This is a field-based role where you will provide one-on-one care and build relationships that allow you to truly see the difference you make. You will deliver skilled nursing services that directly support patient recovery and comfort. The role offers a balanced, flexible work environment with the ability to manage your schedule and workload more independently, working under the guidance of an RN while collaborating with an interdisciplinary care team. Requirements
- Active LPN/LVN license in the state of practice
- Graduate of an accredited practical/vocational nursing program
- Current CPR certification
- Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)
- Strong clinical and observational skills
- Clear communication and patient education abilities
- Ability to follow care plans while recognizing and reporting changes in condition
- Basic proficiency with EMR systems
- Valid driver’s license and reliable transportation
Nice To Haves
- Experience in home health or community-based care
- Strong patient engagement and relationship-building skills
- Ability to work independently while staying connected to a team
- Familiarity with Homecare Homebase (HCHB)
Responsibilities
- Provide direct patient care in accordance with the state Nurse Practice Act
- Implement and support the plan of care developed by the RN
- Deliver skilled nursing services, including treatments, medication support, and wound care
- Monitor patient condition and report changes to the RN and care team
- Educate patients and caregivers on self-care, treatments, and disease management
- Maintain infection control and proper use of equipment and supplies
- Document patient care accurately and timely within the EMR
- Communicate effectively with physicians, nurses, and interdisciplinary team members
- Participate in case conferences, team meetings, and ongoing training
- Support coordinated care by maintaining consistent communication across the care team
