About The Position
Horizon Home Health & Hospice is seeking compassionate nurses with strong clinical skills to join their team in the Emmett, Idaho area. This role covers both Home Health and Hospice services. Requirements
- LPN currently licensed in the State of Idaho
- Must have a current driver's license and auto insurance policy
- Must have a reliable vehicle
Nice To Haves
- Home Health experience is preferred
Responsibilities
- Provides direct patient care as defined in State’s Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor or Director of Nursing.
- Uses equipment and supplies effectively and efficiently.
- Adhering to the organizations documentation and care procedures and standards of personal and professional conduct.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
