About The Position

Attentive Care is a licensed, family- and veteran-owned healthcare staffing agency partnering with skilled nursing facilities, rehabilitation centers, and other healthcare organizations throughout the Capital Region. We are committed to connecting compassionate, dependable LPNs with rewarding opportunities while delivering exceptional care and reliable staffing solutions to our facility partners. We are actively seeking LPNs for current staffing needs at facilities including Glendale Nursing Home in Schenectady, NY, and Daughters of Sarah in Albany, NY. These facilities have significant LPN staffing needs across all shifts (7:00 AM – 3:00 PM, 3:00 PM – 11:00 PM, 11:00 PM – 7:00 AM), with a particular need for weekend availability. Attentive Care is growing its LPN staffing team to meet the needs of its facility partners and to prepare for future opportunities as additional facility needs become available.

Requirements

  • Current New York State LPN license and active registration.
  • Recent experience in skilled nursing, rehab, or long-term care settings.
  • Experience recognizing and reporting changes in resident condition.
  • Eligibility to work in the United States.
  • Reliable transportation.

Nice To Haves

  • Experience with medication administration, wound care, dressing changes, specimen collection, and catheter care.
  • Experience documenting care accurately in EMR systems.
  • Experience supervising CNAs or working closely with RNs, physicians, and therapists.
  • Availability to work weekend, evening, or overnight shifts.

Responsibilities

  • Provide direct nursing care to residents in accordance with individualized care plans, physician orders, and facility policies.
  • Administer prescribed medications, treatments, and delegated nursing procedures accurately and safely while monitoring for effectiveness and adverse reactions.
  • Assess, monitor, and document residents' vital signs, physical condition, and changes in health status, promptly reporting significant findings to the Registered Nurse or attending provider.
  • Perform wound care, dressing changes, specimen collection, catheter care, and other nursing procedures within the LPN scope of practice.
  • Maintain accurate, timely, and complete documentation in the electronic or paper medical record in accordance with regulatory and facility requirements.
  • Supervise and support Certified Nursing Assistants (CNAs) by providing direction, monitoring delegated tasks, and promoting high standards of resident care.
  • Collaborate with Registered Nurses, physicians, therapists, and the interdisciplinary care team to ensure coordinated, high-quality resident care.
  • Respond appropriately to changes in resident condition, medical emergencies, and infection prevention protocols while following established clinical procedures.
  • Promote resident dignity, comfort, safety, and independence while building positive relationships with residents, families, and fellow healthcare professionals.
  • Adhere to all applicable state and federal regulations, facility policies, infection control standards, and patient confidentiality requirements.
