About The Position

Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County. This role offers the opportunity to support patients and families during critical moments by delivering compassionate, high-quality care in the comfort of their homes on the following schedule: 7am - 7:30pm and every other weekend. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

Requirements

Current license to practice as a Licensed Practical Nurse in the State of Florida.
Current Basic Life Support Certification.
Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.

Nice To Haves

Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements preferred
Hospice Certification preferred.

Responsibilities

Support and consistently carry out Trustbridge Hospice’s mission, vision, and values
Work under the direction of the Registered Nurse and Primary Physician to: Identify patient and family needs, Coordinate and deliver appropriate hospice care
Utilize clinical monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life
Provide continuous care focused on symptom management and patient safety
Pledge to make the comfort and care of patients and families the highest priority
Deliver personalized, sincere, and engaging service at all times
Ensure a high level of patient and family satisfaction through compassionate communication and care
