VCU Health is seeking authentic, passionate and inspiring candidates for our virtually enabled Hospital at Home and Transitional Remote Patient Monitoring (tRPM) program. The Enrollment LPN ensures quality successful enrollment of patients into the tRPM program. This includes management of a referral work queue, bedside education on the tRPM program and equipment with patients prior to hospital discharge, and enrolling patients in a the tRPM technology platform. Transitional Remote Patient Monitoring a 30-day post discharge program that supports patients utilizing vital sign monitoring, nurse triage, provider telehealth visits, and nurse navigation. The LPN job commences to integrate technical skills and theoretical knowledge to meet the needs of the patient and family. This role assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. This role also provides direction, education, and begins to assume leadership responsibilities at the unit level. The LPN job delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This role safely administers prescribed medications, identifying incompatibilities and/or interactions.
