About The Position
Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team. This role requires availability of 96 hours per month, including two weekend nights per month, and covers Manatee, Sarasota, Charlotte, and DeSoto counties. The LPN, under physician's orders and RN supervision, is responsible for observing patient/family needs, providing supportive and emotional care during crises to maximize patient comfort and enhance quality of life.
Requirements
- Graduate of an approved nursing program
- Current Florida LPN license
- Minimum one year experience in an acute care setting, providing direct patient care
Nice To Haves
- Previous experience as a hospice LPN preferred
Responsibilities
- Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
- In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
