Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. This role involves working as a member of an inter-professional team, applying a person-centered approach to care. The LPN will complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate and up-to-date, communicating changes as required. They will also implement and evaluate the effectiveness of care/service plans, assist with scheduling and participating in resident's care conferences, and work closely with the Director of Care or Director of Health & Wellness to facilitate resident transitions.
