About The Position

Hocking Valley Community Hospital (HVCH) is seeking a Licensed Practical Nurse (LPN) Case Manager to join their clinical team. This full-time position is for the Case Management/Social Services department, working day shifts, Monday through Friday. The hospital is a full-service critical access hospital located in Logan, Ohio, dedicated to providing high-quality medical services with state-of-the-art equipment and technology. Requirements

Graduate of an accredited program in nursing.
Active State of Ohio Licensed Practical Nurse Nursing License.
At least 5 years of nursing experience in an acute health care setting.
Good interpersonal and written and verbal communication skills.
Computer literacy.
