Centra Rehabilitation Hospital is a brand-new 50 bed inpatient rehabilitation facility opening in 2026. We offer high-quality, compassionate care for a wide variety of patients in need. Requirements

• Graduate of an accredited practical or vocational nursing program.
• Current state LPN/LVN license.
• Current CPR/BCLS certification; ACLS preferred.
• IV certification at hire or within six months.
• Strong communication and interpersonal skills.

Nice To Haves

• Six months of acute care medical-surgical experience preferred.

Responsibilities

• Provide safe, compassionate rehabilitation nursing care within the LPN/LVN scope of practice.
• Administer medications and treatments accurately.
• Assist the RN with assessments and individualized care plans.
• Monitor pain, patient progress and response to treatment.
• Educate patients and families on care needs and treatment plans.
• Collaborate with the interdisciplinary team to coordinate care.
• Promote patient safety, quality improvement and accurate documentation.
• Perform other duties as assigned.
