Licensed Practical Nurse/Licensed Vocational Nurse, Rehabilitation
About The Position
The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient’s needs. This role is crucial in a state-of-the-art, 40-bed inpatient acute rehabilitation hospital dedicated to the treatment and recovery of individuals who have experienced the debilitating effects of a severe injury or illness. The hospital offers customized, individualized rehabilitation tailored to the specific needs of those recovering from a stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury, striving to maximize health, function, and quality of life through comprehensive physical medicine and rehabilitation programs.
Requirements
- LPN license required
- Basic Life Support certification is required within 30 days of hire.
- CPR/BLS certification required.
- IV certification highly preferred or must be obtained within 6 months.
Nice To Haves
- Minimum of six months of Medical/Surgical experience in an acute care setting preferred
Responsibilities
- Administer medications safely, accurately, and based on patient age and clinical condition.
- Minimize potential for medication errors by following proper medication administration procedures.
- Assist the RN with initial assessments and patient reassessments per policy.
- Identify and implement appropriate rehabilitation nursing interventions.
- Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
- Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
- Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
- Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
- Develop and implement individualized teaching plans based on patient and family learning needs.
- Perform other duties as assigned.
