About The Position

This unit has 60 beds, with an average occupancy of 50 to 53 residents. We care for patients with diagnoses such as Dementia, Alzheimer's, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and those in need of short-term rehabilitation. In addition, there are 26 private rooms. Our primary focus is on the senior population and conveniently located within Person Memorial Hospital (PMH). Nurses will collaborate closely with the laboratory, rehabilitation, hospice, and psychiatric teams.

Requirements

  • Current state Licensed Practical Nurse (LPN) license
  • High school diploma or GED equivalent
  • Basic Life Support certification from the American Heart Association is required upon hire.

Responsibilities

  • Under the supervision of an RN, carries out and documents assigned tasks and nursing interventions as addressed in the plan of care.
  • Reviews and revises the plan of care.
  • Monitors each patient for the physical and psychosocial needs and alters plan of care as discussed with RN.
  • Prepares patients for OR and special procedures.
  • Administers medication by all routes, performs venipuncture and IV therapy.
  • Directs and assists in the supervision of Patient Care Techs.
  • Provides nutritional support as necessary (oral, enteral, peripheral) except Hyperalimentation.
  • Evaluates, communicates and documents care delivered to other members of the health care team through shift report, nursing care plans, progress notes, Nursing notes, direct communications to team members, etc.
  • Inserts, monitors and maintains NG tubes, urinary caths, etc.
  • Assists patient with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Assists physicians with exams, procedures, dressing changes, etc. as necessary.
  • Receives physicians’ orders, enters into the electronic medical record, and completes.
  • Assists with discharge instruction to patient and/or family regarding necessary information to continue care at home and documents response.
  • Recognizes emergency situations and responds appropriately.
  • Participates in code situations as directed.
  • Reviews and performs shift assessments and reports any variations/concerns to RN.
  • Gather patient history and medication information.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • May be required to perform other assignments from time to time as requested by the Department Leader.
