The Licensed Practical Nurse Lead is responsible for non-clinical tasks, direct patient care, ongoing education, reinforcement of care plans, and providing general support in an ambulatory setting under the direction and supervision of the patient’s physician or licensed medical practitioner or designee in accordance with policy and procedure and competency to promote patient health and wellness.

The Licensed Practical Nurse Lead serves as a clinical leader and serves as a resource for CMA and LPN team members, providing expertise and guidance as well as validating skills and competencies.

This position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.

Requirements

• High School Diploma or GED required.
• Graduate of an accredited practical nursing program required.
• Minimum of 2 years experience in a clinic setting required.
• Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding.
• AHA Healthcare Provider BLS required.
• Ability to successfully complete general and department role based specific skills validation and competency testing.

Responsibilities

• Responsible for non-clinical tasks, direct patient care, ongoing education, reinforcement of care plans, and providing general support in an ambulatory setting under the direction and supervision of the patient’s physician or licensed medical practitioner or designee in accordance with policy and procedure and competency to promote patient health and wellness.
• Serves as a clinical leader and serves as a resource for CMA and LPN team members, providing expertise and guidance as well as validating skills and competencies.
• Practicing safely within the defined standards of practice.
• Records and relays patient information, symptoms, and concerns to the patient’s medical provider or registered nurse for further triage assessment and to determine a plan of care.
• Assist patients with therapeutic regimens.
• Reinforces established plans of care and age-appropriate interventions.
• Reinforces patient/family teaching plan based on identified health education needs and the condition and age of the patient through approved educational materials or written instructions.
• Administer tasks, treatments, and medications in accordance with validated competencies.
• Communicates effectively and professionally with all members of the interdisciplinary team, patients, medical providers and all levels of healthcare.
• Conveys information accurately to physicians, nurses, and other team members, as well as listen and respond to the needs and concerns of patients and colleagues.
• Utilizes appropriate communication tools when communicating within the interdisciplinary team.
• Documents patient information in EHR.
• Willing to share responsibilities, support their teammates, and work together to achieve common goals.
• Organizes and prioritizes daily work assignments to support patient needs, the interdisciplinary team, and operational efficiency.
• Demonstrates teamwork in the delivery of patient care. Recognizes the importance of time sensitive issues.
• Practices within the boundaries of the regulations governing the practice of nursing in the Commonwealth of Virginia.
• Engages in professional development activities to stay current with advancements and best practices in ambulatory care.
• Possesses clinical knowledge and skills to meet standards as required by specific clinical areas.
• Contributes to the development of practice guiding resources (policies, procedures, guidelines, protocol orders, etc.).
