The Licensed Practical Nurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. In addition, the incumbent will provide advocacy and liaison with community health care providers, monitoring medical records, and medication administration. The schedule is Monday-Friday 9:00 am-3:00 pm, totaling 30 hours per week. This role involves supporting individuals with all activities of daily living, including personal care, monitoring vital signs, and ensuring their comfort, privacy, and dignity. The LPN will observe residents for changes in condition or behavior, report these changes promptly, and provide services according to regulatory standards. This includes assisting with medical, clinical, and financial needs, transporting individuals to appointments and activities, and assisting with housekeeping. The role also requires maintaining progress notes, writing annual assessments, and participating in Individual Service Plan (ISP) meetings. Core training certifications, including CPR/FA, OSHA, and PABC, must be maintained, along with MAP training.
