Licensed Practical Nurse Intake Coordinator
About The Position
Sentara Home Care is seeking to hire an LPN Intake Coordinator. This is a flexible position with weekend hours as needed, primarily remote with occasional onsite meetings and trainings in Chesapeake, VA. The eligible candidate must live no more than one hour away from Chesapeake, VA. The hourly range for this position is $17.31 - $28.80. The role involves evaluating and processing referrals by receiving and responding to incoming calls from referral sources or potential patients. The coordinator will record the outcome of calls, make follow-up calls as necessary, and enter referral information into the system, including clinical information and orders for services. A working knowledge of all services and resources provided by Sentara and available within the community is essential, and the role will act as a resource for patient and referral source issues.
Requirements
- Highschool Graduate or Equivalent
- LPN- Active in Virginia
- Minimum 1 year of experience with clinical and customer service in a healthcare environment
- Eligible candidate must live no more than one hour away from Chesapeake, VA.
Nice To Haves
- Strong care coordination experience is highly preferred
- Home Health experience is highly preferred
