Licensed Practical Nurse, Hospice
About The Position
Join our team as a Licensed Practical Nurse (LPN), where you'll provide compassionate, person-centred care while working collaboratively with an interdisciplinary healthcare team. In this role, you'll assess, plan, implement, and evaluate resident/patient care, advocate for individual needs, and support overall health and well-being. You'll practice to the full scope of LPN standards, contribute to care planning, mentor new staff and students, and make a meaningful impact in a supportive and collaborative environment. Minimum Nurse-to-Patient Ratio Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care. Requirements
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
- Current basic cardiac life support certification.
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
- Ability to communicate effectively both verbally and in writing.
- Ability to deal with others effectively.
- Physical ability to carry out the duties of the position.
- Ability to operate related equipment.
- Ability to work as a member in an interdisciplinary team.
- Ability to apply decision making and analytical skills.
- Ability to role model conflict resolution strategies.
- Ability to prioritize and organize work.
- Ability to teach and identify patient/resident learning needs.
Responsibilities
- Deliver safe, compassionate, person-centred nursing care in accordance with LPN standards and individualized care plans.
- Perform comprehensive patient/resident assessments, monitor changes in condition, and communicate concerns to the care team.
- Administer medications (excluding IV), monitor therapeutic effects, and carry out physician orders accurately.
- Develop, implement, evaluate, and document nursing care plans and interventions.
- Collaborate with physicians and the interdisciplinary team to support quality patient/resident care and discharge planning.
- Prepare and support patients/residents for admissions, transfers, procedures, and discharges.
- Educate patients/residents and families, promote informed decision-making, and advocate for their physical, emotional, cultural, and spiritual needs.
- Respond appropriately to emergencies, incidents, and safety concerns while following established reporting procedures.
- Mentor and support new staff and students by sharing clinical knowledge and best practices.
- Participate in care conferences, team meetings, quality improvement initiatives, and maintain a safe, organized care environment.
