The Home Health Care LPN will play a critical role in providing quality medical care and support to individuals in their homes. The primary responsibility will be to perform skilled nursing interventions, educate patients on how to manage their health conditions, administer medications, and collaborate with other healthcare professionals to ensure comprehensive care. The Home Health LPN plays a critical role in delivering skilled nursing care and comprehensive case management services to patients in their homes. In this role, the LPN functions as both a clinician and a case manager—conducting holistic assessments, developing and managing individualized plans of care, coordinating services across the interdisciplinary team, and monitoring patient progress throughout the episode of care. The Home Health LPN serves as the primary point of clinical oversight, ensuring care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals, while supporting successful outcomes and transitions across the continuum of care.
