About The Position
Home Care Practical Nurse (LPN) Mercy Home Health Make a meaningful impact—one patient at a time. As a Home Care Practical Nurse (PN) with Mercy Home Health, you’ll bring high-quality, compassionate care directly to patients where they feel safest and most comfortable: their homes. You’ll use advanced tools, your clinical expertise, and a whole-person approach to help patients heal, regain independence, and thrive. This position is based out of Newtown Square and will support Delaware County.
Requirements
- Graduate of an accredited nursing program
- Active LPN license in the State of Pennsylvania
- At least one (1) year of professional nursing experience preferably in Home Health
- Phlebotomy and wound care experience required
Responsibilities
- Deliver high-quality, compassionate care directly to patients in their homes.
- Utilize advanced tools, clinical expertise, and a whole-person approach to patient care.
- Help patients heal, regain independence, and thrive.
- Provide skilled nursing, therapy services (physical, occupational, speech), and medical social work.
- Streamline charting and communication with the Epic EMR System.
