About The Position

This position is for a Licensed Practical Nurse in Home Health, serving the Sumter/Clarendon area. The role involves assisting with revisits within a geographical area, working under the direction of the primary RN and in collaboration with other caregiver staff (PT, OT, ST, MSW) for interdisciplinary patient care. The nurse is responsible for independent actions consistent with professional nursing standards and must demonstrate strong communication, interpersonal, and organizational skills. The position requires the ability to handle safety issues, understand growth and development across the lifespan, and provide age-specific care. A high degree of maturity, self-motivation, creativity, initiative, and independent judgment are essential, along with responsible, ethical, and self-initiating professional behavior. Effective interpersonal skills, empathy, and the ability to listen and plan with staff, patients, customers, and physicians are crucial. The nurse must be able to communicate efficiently with a diverse population and exhibit self-awareness, self-evaluation, accuracy in data collection, and follow-up. Close collaboration with other staff, physicians, and external agencies is expected. Adherence to dress code, punctuality, and attendance policies is required.

Requirements

  • At least one year of nursing experience required.
  • Must possess a current valid SC driver's license and automobile insurance as required by law.
  • Must be at least 21 years old to be covered under McLeod Health's fleet vehicle insurance policy.
  • Must meet McLeod policy for driving eligibility.
  • License and registration as required by state law.
  • LPN License.
  • BLS Certification (American Heart Association).

Nice To Haves

  • One year home care highly desirable.
  • Previous home care experience preferred.
  • Good writing skills and computer literacy are desirable.
  • Organizational and time management skills desirable.
  • Possession of professional liability insurance is encouraged.

Responsibilities

  • Assisting with revisits within a geographical area.
  • Working under the direction of the primary RN.
  • Collaborating with all caregiver staff (team RN, PT, OT, ST, MSW) for interdisciplinary coordination of patient care.
  • Performing independent actions consistent with the professional standards of nursing practice.
  • Handling potential problems related to safety issues for staff and patients in all settings.
  • Assessing data reflective of the patient's status and interpreting information needed for each patient's requirements relative to his/her age-specific needs to provide the care needed.
  • Communicating efficiently with a diverse population.
  • Collecting data accurately and completing records and follow-up.
