The Medical-Surgical nurse is responsible for managing the care of the adult or geriatric patient experiencing general medical conditions or general surgical procedures. The nurse must be able to assess patient condition, administer medications, change dressings, monitor vital signs, keep records and provide patients and families with support and education. The Medical-Surgical nurse maintains a wide array of medical care knowledge in order to care for a diverse group of patients. The Licensed Practical Nurse working in the Transitional Care Unit is responsible to provide direct care of the adult/geriatric patient experiencing prolonged general medical or post-operative recovery. The LPN provides care based on the patients’ needs, which includes medication administration, dressing changes, monitoring of vital signs and skin integrity, documentation in the medical record, and providing patients and families with support and education. This role is part of an interdisciplinary team that is patient-focused, working daily with all types of adult patients from cardiac to trauma to ortho and neuro. The focus is on the patient returning home at their maximum level of abilities. As a nurse, you have frequent interactions with the patient and family providing immediate care and education for a successful transition home.
