About The Position

The primary purpose of this role is to provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants. This supervision must align with the Florida Nurse Practice Act, current federal, state, and local standards, guidelines, and regulations governing the facility, and as directed by the Director of Nursing or Nurse Supervisor/Unit Manager to ensure the highest degree of quality care is maintained at all times. The Licensed Practical Nurse/ Floor Nurse is delegated the administrative authority, responsibility, and accountability necessary for carrying out assigned duties.

Requirements

  • Graduate of an accredited school of nursing.
  • Must possess and maintain an “Eligible” ACHA Background Screening status.
  • Valid and current license as a Licensed Practical Nurse in the State of Florida.
  • Excellent communication and interpersonal skills.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must possess leadership ability and the willingness to work harmoniously with and supervise other personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
  • Must be able to move intermittently throughout the work day.
  • Must be able to speak, read and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately.

  • Must meet the general health requirements set forth by the policies of this facility.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
  • Must be able to twist and turn all extremities and at the waist.
  • Must be able to push, pull, move, and/or lift a minimum of 35 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 450 feet.

Nice To Haves

Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines.
  • Ensure that all nursing personnel assigned comply with facility policies and procedures.
  • Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
  • Participate and cooperate in facility surveys (inspections) by local, state, and federal agencies.
  • Admit, transfer, and discharge residents in accordance with facility protocols.
  • Make written and oral reports/recommendations concerning the activities of your shift.
  • Cooperate with other resident services/departments when coordinating nursing services.
  • Periodically review and assist in updating the resident's written discharge plan.
  • Complete accident/incident reports and submit to the Director of Nursing or designee.
  • Complete and file required recordkeeping forms/charts upon resident's admission, transfer, and/or discharge.
  • Receive telephone orders from physicians and record in the EMR system.
  • Transcribe physician's orders to resident’s EMR, card-ex, medication cards, and treatment care plans.
  • Chart nurses' notes in an informative and descriptive manner reflecting care provided and resident's response.
  • Chart all reports of accidents/incidents involving residents.
  • Record new/changed diet orders and forward information to the Dietary Department.
  • Report discrepancies concerning physician's orders, diet changes, charting errors, etc., to the Nurse Supervisor/Unit Manager.
  • Fill out and complete transfer forms.
  • Ensure appropriate documentation concerning unauthorized discharges is entered in the resident's medical record.
  • Perform routine charting duties as required.
  • Prepare and administer medications as ordered by the physician.
  • Verify the identity of the resident before administering medication/treatment.
  • Ensure prescribed medication for one resident is not administered to another.
  • Ensure an adequate supply of floor stock medications, supplies, and equipment is on hand.
  • Order prescribed medications, supplies, and equipment as necessary.
  • Ensure that narcotics are handled in accordance with federal, state, and local laws and facility policies.
  • Ensure that narcotic records are accurate for your shift.
  • Notify the Nurse Supervisor/Unit Manager of all drug and narcotic discrepancies.
  • Review medication cards for completeness, accuracy, and adherence to stop order policies.
  • Notify the attending physician of automatic stop orders prior to the last dosage being administered.
  • Dispose of drugs and narcotics as required.
  • Participate in employee performance evaluations and determine staffing requirements.
  • Make recommendations to the Nurse Supervisor/Unit Manager concerning employee dismissals, transfers, etc.
  • Inform the Nurse Supervisor of staffing needs when assigned personnel fail to report to work.
  • Report absentee call-ins to the Nurse Supervisor/Unit Manager.
  • Develop work assignments and/or assist in completing and performing such assignments.
  • Monitor Nursing Assistants to ensure care and services are provided according to the resident’s plan of care.
  • Report problem areas to the Nurse Supervisor/Unit Manager.
  • Develop and maintain good working rapport with inter-department personnel.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis.
  • Review complaints and grievances made or filed by assigned personnel and make appropriate reports.
  • Receive/give the nursing report upon reporting in and ending shift duty hours.
  • Report occupational exposures to blood, body fluid, infectious materials, and hazardous chemicals.
  • Inform nursing service personnel of new admissions, their expected time of arrival, and room assignment.
  • Ensure that rooms are ready for new admissions.
  • Greet newly admitted residents and escort them to their rooms.
  • Observe and document the status of each patient upon admission and throughout treatment.
  • Report concerns to the Registered Nurse for assessment.
  • Participate in the orientation of new residents/family members to the facility.
  • Make rounds with physicians as necessary.
  • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician.
  • Consult with the resident's physicians regarding resident care, treatment, and rehabilitation.
  • Review the resident's chart for specific treatments, medication orders, diets, etc.
  • Implement and maintain established nursing objectives and standards.
  • Make periodic checks to ensure prescribed treatments are properly administered and to evaluate resident's physical and emotional status.
  • Ensure that direct nursing care is provided in accordance with the resident’s plan of care and wishes.
  • Notify the resident's attending physician when the resident is involved in an accident or incident.
  • Notify the resident's attending physician and next-of-kin when there is a change in the resident's condition.
  • Carry out restorative and rehabilitative programs.
  • Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, range of motion exercises, and care for the dead/dying.
  • Use restraints only when prescribed and in accordance with established policies and procedures.
  • Obtain sputum, urine, and other specimens for lab tests as ordered.
  • Take and record TPRs, blood pressures, etc.
  • Monitor seriously ill residents as necessary.
  • Check food brought into the facility by resident's family/visitors.
  • Promptly respond to call bells.
  • Promptly and courteously respond to requests for assistance.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Meet with residents, and/or family members, as necessary.
  • Admit, transfer, and discharge residents as necessary.
  • Assist in arranging transportation of discharged residents.
  • Ensure that discharged residents are escorted to the pick-up area.
  • Inform family members of the death of the resident.
  • Call funeral homes when requested by the family.
  • Ensure that established post-mortem procedures are followed.
  • Act in compliance with established regulatory and professional standards and guidelines.
  • Attend and participate in all facility in-service trainings/programs.
  • Attend and participate in annual OSHA and CDC in-service training programs.
  • Attend and participate in advance directive in-service training programs.
  • Attend and participate in continuing education programs.
  • Monitor assigned personnel to ensure they are following established safety regulations.
  • Ensure that established departmental policies and procedures are followed.
  • Ensure that your assigned work areas are maintained in a safe, clean, and sanitary manner.
  • Ensure that your unit's resident rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
  • Ensure that you and your assigned personnel follow established safety policies and procedures.
  • Ensure that you and your assigned personnel follow established hand-washing procedures.
  • Ensure that you and your assigned personnel participate in and conduct all fire safety and disaster preparedness drills.
  • Ensure that you and your assigned personnel follow established infection control procedures when isolation precautions become necessary.
  • Ensure that you and your assigned personnel follow established infection control procedures.
  • Ensure that you and your assigned personnel wear and/or use safety equipment and supplies when moving or lifting a resident.
  • Report missing/illegible labels and MSDSs to the safety officer or other designated person.
  • Recommend equipment and supply needs of your unit/shift.
  • Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained.
  • Ensure that only trained and authorized personnel operate your unit/shift's equipment.
  • Ensure that all personnel operate nursing service equipment in a safe manner.
  • Review care plans daily to ensure appropriate care is being rendered.
  • Inform the Nurse Supervisor/Unit Manager of any changes that need to be made on the care plan.
  • Ensure that your nurses' notes reflect that the care plan is being followed.
  • Review resident care plans for appropriate resident goals, problems, approaches, and revisions.
  • Ensure that your assigned LPNs and CNAs are aware of the resident care plans and carry out the resident's care plan.
  • Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS.
  • Maintain the confidentiality of all resident care information.
  • Monitor nursing care to ensure that all residents are treated fairly, and with kindness, dignity, and respect.
  • Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room.
  • Ensure that all nursing service personnel are knowledgeable of resident rights and responsibilities.
  • Review complaints or grievances made by the resident and make a written/oral report to the Nurse Supervisor/Unit Manager.
  • Maintain a written record of the resident's complaints and/or grievances.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor the resident's refusal of treatment request.
  • Adhere to facility and departmental policies and procedures.
  • Perform other duties as assigned.
  • Assist in the evacuation of residents during emergency situations.
