Licensed Practical Nurse - Dialysis - LPN

This role involves providing nursing care to assigned patients under the direct supervision of a registered nurse (RN) or other assigned supervisor, collecting patient data, and participating as a member of the health care team. The LPN delivers care in accordance with the patient care plan and organizational policies, utilizing strong communication, mentoring, and instructional skills. Participation in committees and quality improvement projects may also be involved.
Requirements
- Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
- If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
- Excellent communication skills (verbal and written).
- Experience working in a team environment.
- Computer skills required.
- Ability to work daytime hours, flexibility may be required to meet staffing needs.
- Ability to adapt to unpredictable situations within the work setting.
- Current LPN license by applicable state requirements.
- Maintains current Basic Life Support (BLS) for Health Care Providers (Arizona and Florida: Basic Life Support (BLS) for Health Care Providers; Midwest: Basic Life Support for Health Care Providers from American Heart Association or American Red Cross).
- Additional specialty certification/training as required by the work area.
Nice To Haves
- One year of LPN experience within the last five years preferred.
- Prior experience with electronic medical record systems preferred.
- Graduate of an accredited school of practical nursing preferred.
Responsibilities
- Provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor.
- Accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.
- Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
- Participates as a member of the health care team.
- Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
- Possesses excellent communication skills; is skillful in mentoring and instructing.
- May participate on committees or projects, including quality improvement projects.
