About The Position

This PRN (As Needed) position requires a Licensed Practical Nurse with critical thinking skills to detect changes in resident condition and follow protocol for urgent and emergent care. The role involves providing quality immediate care to residents who are medically unstable or injured, demonstrating good problem-solving and decision-making skills. Responsibilities include reporting abnormal findings to the Clinic Manager, meeting with resident family members as necessary, maintaining records of treated residents, preparing incident reports, and notifying Family, POA, and MD. The LPN will collaborate with the Interdisciplinary Team (IDT) to ensure appropriate follow-up and support, handle queries from residents, families, and MDs after a medical incident, and demonstrate abilities to plan and organize workload. Excellent verbal and written communication skills, maintaining confidentiality of resident care information, and promoting excellent customer service to create a positive work environment are essential. The role also involves promoting Associate and Resident satisfaction, monitoring and collaborating with the Mender’s By Lenbrook Team to ensure assignments are carried out as scheduled, and medication oversight. Other duties as assigned.

Requirements

  • Current Licensed Practical Nurse License
  • Minimum of 3- 5 years nursing experience in Ambulatory Care or similar setting.
  • CPR Certification
  • Must have critical thinking skills to detect changes in condition and follow protocol for urgent and emergent care.
  • Must provide quality immediate care to Residents who are medically unstable or injured.
  • Demonstrate good problem solving and decision-making skills.
  • Ability to communicate effectively with staff, residents and families.
  • Ability to make independent decisions when circumstances warrant.
  • Must possess ability to deal tactfully with personnel, residents, family members, visitors, and the general public.
  • Ability to relate information concerning a resident’s condition.
  • Ability to supervise and manage resident care.
  • Strong verbal and written communications skills.
  • Ability to work in a fast-paced environment with demonstrated ability to juggle multiple tasks and demands.

  • Knowledge of federal and state regulations for long-term care, assisted-living and continuing-care facilities.

Responsibilities
