About The Position

The Licensed Practical Nurse (LPN) Charge Nurse supervises, provides, and directs nursing care in compliance with company and facility policies and procedures. This role involves making advanced observations, reporting and recording them appropriately, forming nursing judgments, and carrying out nursing actions based on scientific principles. The LPN Charge Nurse ensures compliance with all applicable federal, state, and local regulations, interprets policies for staff, families, and physicians, and develops comprehensive care plans based on resident needs. They are responsible for medication management, staffing, monitoring resident diets, and adhering to facility admission procedures.

Requirements

Current Nurse License
Graduate of an accredited school of nursing or a vocational school of practical nursing
Working knowledge of Windows, Microsoft Office Suite and the Internet
Good written and verbal communication skills
Good communication, comprehension and interpersonal skills
Ability to speak, read, write and understand English

Responsibilities

Supervise, provide, and direct nursing care in compliance with facility policies and procedures.
Make advanced observations, report and record them appropriately, form nursing judgments, and carry out nursing actions.
Follow established standards of nursing practices and assume responsibility for compliance with federal, state, and local regulations.
Interpret existing policies to staff, families, and physicians.
Perform daily rounds and develop/implement accurate comprehensive care plans based on each resident’s needs.
Follow established policies and procedures for passing medications and treatments, and manage medication inventory.
Follow staffing guidelines and make nursing assignments.
Receive and give accurate reports on resident's condition to ensure continuity of care.
Monitor diets and supervise meal trays and snacks served to residents, assisting as needed.
Follow facility admissions procedures and communicate admissions to relevant parties.
Ensure employees follow the chain of command and assign nursing assistants, ensuring efficient completion of their assignments.
Supervise, document, and correct poor performance of nursing assistants, keeping Unit Managers/Director of Nursing informed.
Participate in the performance evaluation process of nursing assistants, providing counseling, coaching, and training.
Communicate professionally and effectively, following written and verbal directions.
Provide exemplary customer service to residents, peers, families, managers, and visitors.
Report any changes in physical or mental conditions on a timely basis.
Follow all company, facility, and state policies and procedures.
Attend staff meetings, in-services, and other assigned meetings.
Exercise keen observations of environmental conditions and cooperate with other staff members.
Document resident information timely, accurately, and confidentially.
Verify change-over MARS/TARS are correctly updated.
Adhere to the guidelines outlined in the Resident Rights documentation, ensuring residents' rights are protected.
Report resident complaints to the supervisor in charge and maintain confidentiality.
Adhere to all policies and procedures outlined under Safety & Compliance.
Ensure residents are safe, including proper use of equipment like Wanderguard.
Demonstrate proper use of equipment, report equipment needs or repairs.
Use protective equipment, follow infection control protocol standards, policies, and procedures.
Practice universal precautions and life safety measures.
Complete all annual education requirements timely.
Perform other duties as assigned.
