About The Position
The Licensed Practical Nurse (LPN) – Care Coordinator serves a critical role in an integrated care management model, delivering clinical care coordination and telephonic Chronic Care Management (CCM) under general practitioner supervision. This is a remote/hybrid position.
Requirements
- Current State of Hawaii Licensed Practical Nurse (LPN) licensure in good standing.
- Proficiency in basic computer skills including but not limited to, navigating web browsers, electronic medical record systems, computer systems and applications, Microsoft office suite.
Nice To Haves
- Experience in healthcare or home health field or work with the geriatric population.
Responsibilities
- Conduct structured telephone interactions with enrolled CCM patients to assess health status, symptom control, and care plan progress.
- Assist in updating and maintaining comprehensive, patient-centered electronic care plans in the EHR following CMS clinical guidelines.
- Document all eligible non-face-to-face clinical care management time to support CCM billing requirements.
- Perform routine telephonic medication reviews, identify gaps in adherence or refill barriers, offer patient education, and escalate complex drug-related problems to the supervising provider.
- Monitor patient-reported symptoms and vitals via telephone; identify early signs of clinical decompensation and escalate to treating providers as appropriate.
