About The Position

The Licensed Practical Nurse is responsible for providing direct patient care, ongoing education and reinforcement of care plans, and providing general support in an ambulatory setting under the direction and supervision of the patient’s physician or licensed medical practitioner or designee in accordance with policy and procedure and competency to promote patient health and wellness. This position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.

Requirements

  • High School Diploma required
  • Graduate of accredited Practical Nursing Program required
  • Completion of a nursing program as approved by the State Board of Nursing
  • Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding
  • AHA Healthcare Provider BLS required
  • Ability to successfully complete general and department role based specific skills validation and competency testing

Nice To Haves

  • Minimum of 1 year of nursing experience preferred

Responsibilities

  • Providing direct patient care
  • Ongoing education and reinforcement of care plans
  • Providing general support in an ambulatory setting
  • Practicing safely within the defined standards of practice
  • Recording and relaying patient information, symptoms, and concerns to the patient’s medical provider or registered nurse for further triage assessment and to determine a plan of care
  • Assisting patients with therapeutic regimens
  • Reinforcing established plans of care and age-appropriate interventions
  • Reinforcing patient/family teaching plan based on identified health education needs and the condition and age of the patient through approved educational materials or written instructions
  • Administering tasks, treatments, and medications in accordance with validated competencies
  • Communicating effectively and professionally with all members of the interdisciplinary team, patients, medical providers and all levels of healthcare
  • Conveying information accurately to physicians, nurses, and other team members, as well as listening and responding to the needs and concerns of patients and colleagues
  • Utilizing appropriate communication tools when communicating within the interdisciplinary team
  • Documenting patient information in EHR
  • Willing to share responsibilities, support their teammates, and work together to achieve common goals
  • Organizing and prioritizing daily work assignments to support patient needs, the interdisciplinary team, and operational efficiency
  • Demonstrating teamwork in the delivery of patient care
  • Recognizing the importance of time sensitive issues
  • Practicing within the boundaries of the regulations governing the practice of nursing in the Commonwealth of Virginia
  • Engaging in professional development activities to stay current with advancements and best practices in ambulatory care
  • Possessing clinical knowledge and skills to meet standards as required by specific clinical areas
  • Contributing to the development of practice guiding resources (policies, procedures, guidelines, protocol orders, etc.)
  • May be asked to perform additional duties as assigned
