About The Position

We are seeking a compassionate and organized LPN to join our Admissions team. This role will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and Assisted Living Facilities. This role focuses on discharge planning (DCP) for patients transitioning from hospitals or rehabilitation facilities to hospice care. The ideal candidate will be comfortable working independently, collaborating with interdisciplinary teams, and supporting patients and families during an important stage of care.

Requirements

  • Current Licensed Practical Nurse (LPN) licensure (in good standing) in the state(s) of practice with one year of experience under license
  • Ability to work well within a team environment to collaborate daily with a team of clinicians
  • Work closely with patients on a 1-1 level but get strong support from a patient care team
  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of state and federal rules and regulations
  • Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
  • Ability to learn an electronic medical records system
  • Must be willing to drive and reliable transportation, valid driver’s license, and auto insurance

Responsibilities

  • Travel to hospitals and rehabilitation facilities to meet with patients and families to discuss hospice services and care options
  • Collaborate with hospice physicians to evaluate and determine hospice eligibility
  • Coordinate and communicate discharge plans with facility staff and internal care teams
  • Provide education and emotional support to patients and families throughout the admission and discharge process
  • Complete timely and accurate documentation related to admissions and discharge planning
  • Assist clinical teams with routine or symptom management visits when discharge planning visits are not scheduled
  • Maintain effective communication with referral sources, case managers, and interdisciplinary team members
