About The Position

Relient Health is seeking licensed Physical Therapists (PTs) for full-time, in-person opportunities within established rehabilitation departments across California. These are full-time, direct-hire, permanent placement PT positions. Our openings include opportunities in Skilled Nursing Facilities (SNF), acute care hospitals, inpatient rehab facilities (IRF), outpatient clinics, and home health settings depending on the specific location. Current PT openings are available in Atwater, Carmichael, Fresno, Hayward, Lompoc, Los Angeles, Pleasant Hill, Reseda, Riverside, San Jose, San Rafael, and additional California markets. This role is specific to licensed Physical Therapists working within a rehabilitation department and is not a CNA, nursing assistant, medical assistant, or therapy aide position.

Requirements

Graduate of an accredited Doctor of Physical Therapy (DPT) program
Active California PT license, or eligibility/pending licensure
Ability to work full-time in one of the listed California markets
Authorized to work in the United States

Responsibilities

Evaluate patients and develop individualized treatment plans
Provide skilled physical therapy services in a skilled nursing, hospital/acute, inpatient rehab (IRF), outpatient, or home health environment
Collaborate with interdisciplinary rehab and nursing/medical teams
Supervise and partner with Physical Therapist Assistants (PTAs)
Document care in EMR systems per state, federal, and facility standards
Promote functional independence, safety, and quality of life
