About The Position

HealthPro Heritage has a great full-time Licensed Physical Therapist Assistant (LPTA) opportunity near Boulder, CO. This role involves providing skilled therapy services to residents in Independent living, Assisted living, Memory Care, and Skilled Nursing Community settings. The facilities are beautiful and include a high-functioning outpatient clinic. While outpatient is typically busier, the Long Term Care side is also experiencing growth.

Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
