Home Health Care, Inc. is seeking a Licensed Physical Therapist Assistant (PTA) to help patients achieve their goals in their everyday lives. This role offers one-on-one patient care, functional therapy in the patient's home environment, and greater flexibility and independence. The company has been helping patients recover safely at home for over 30 years and provides a supportive environment for clinicians.
Requirements
- Active Minnesota Licensed Physical Therapist Assistant (PTA) license.
- Compassionate, patient-centered approach to care.
- Strong communication and organizational skills.
- Valid driver's license and reliable transportation.
Nice To Haves
- Home health experience is helpful but not required.
Responsibilities
- Deliver therapeutic interventions established by the supervising Physical Therapist.
- Assist patients with strengthening, balance, gait training, transfers, and mobility exercises.
- Reinforce home exercise programs and educate caregivers.
- Monitor patient progress and communicate updates to the supervising PT.
- Collaborate with physicians and the interdisciplinary care team.
- Complete accurate, timely documentation.
