About The Position

HealthPro Heritage has a great Licensed Physical Therapist Assistant (LPTA) Opportunity at our outstanding Assisted Living and Memory Care facility at Middlebrook Farms and Sturges at Fairfield in Trumble and Fairfield, Connecticut. This role will float between Middlebrook Farms and Sturges at Fairfield. HealthPro Heritage is a therapist-led, diverse organization providing clinical services across various settings, including nursing facilities, retirement communities, hospitals, home care, and pediatric schools and clinics.

Requirements

Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
